At AmpleHealth, our Nephrology, Dialysis and Hypertension Program provides comprehensive, patient-centered care for adults with kidney disease, difficult-to-control hypertension and related medical conditions.

Our goal is to identify kidney problems early, slow the progression of chronic kidney disease and help each patient make informed decisions about treatment. We coordinate closely with primary care physicians, specialists, hospitals and dialysis providers to deliver seamless care across every stage of kidney disease.

Chronic kidney disease management

Kidney disease may progress silently, with few symptoms during its early stages. Timely evaluation and ongoing monitoring can help preserve kidney function and reduce the risk of complications.

Hypertension care

High blood pressure is both a major cause and a common complication of kidney disease. Our team evaluates the underlying factors that may contribute to uncontrolled or resistant hypertension and develops an individualized treatment plan.

Care may include medication optimization, laboratory and diagnostic evaluation, lifestyle recommendations and assessment for secondary causes of hypertension. Our objective is to improve blood-pressure control while protecting the kidneys, heart and blood vessels.

Dialysis and advanced kidney care

For patients with advanced kidney disease, we provide education and guidance regarding kidney replacement therapy so that patients and their families can make informed decisions before dialysis becomes necessary.

Whenever appropriate, we support home-based dialysis options that can provide greater independence and flexibility. Treatment recommendations are tailored to each patient's medical condition, lifestyle, support system and preferences.

Led by Rekha Pareek, MD

AmpleHealth's Nephrology, Dialysis and Hypertension Program is spearheaded by Rekha Pareek, MD, a board-certified nephrologist and internist with more than 20 years of experience providing inpatient and outpatient kidney care.

Dr. Pareek practiced with Kaiser Permanente Medical Group from 2005 through 2025, caring for patients with chronic kidney disease, hypertension, diabetes, acute kidney injury and complex electrolyte disorders. She has extensive leadership experience in home hemodialysis, peritoneal dialysis and therapeutic plasmapheresis.

For 15 years, Dr. Pareek served as Medical Director of the Fresenius Home Dialysis Unit in Roseville, where she led clinical quality initiatives and helped expand access to home dialysis. She also served as Medical Director of the Kaiser Roseville and Kaiser Morse Avenue plasmapheresis programs, Departmental Lead for Peritoneal Dialysis and a regional workgroup member for home dialysis therapies.

Since October 2025, Dr. Pareek has served as Program Director of Nephrology at Sacramento Rehabilitation Hospital, where she established the hospital's first inpatient dialysis program and continues to provide comprehensive inpatient nephrology care.

Dr. Pareek completed her nephrology fellowship at Drexel University College of Medicine, where she served as Chief Nephrology Fellow. She is board certified in nephrology and internal medicine and was recognized by her physician peers as a Sacramento-area Top Doctor in Nephrology by Sactown Magazine in 2025 and 2026.
