When a hospital stay is involved
A hospital admission carries more risk for an older adult, and the days afterward carry more still. We have particular experience with patients admitted after a fall or a sudden illness, and with those whose long-standing conditions have destabilized.
Because we provide hospital care and post-acute care ourselves, that whole stretch stays with one team: the admission, the stay, the discharge, and the rehabilitation that follows.
Memory, cognition, and the people alongside you
We regularly care for patients living with Alzheimer's disease, and we work with the families carrying it alongside them. Much of the practical weight of a dementia diagnosis falls on relatives, and they need a clinician they can reach as much as the patient does.
