Recovery after a hospital stay is a vulnerable window. We manage transitions through skilled nursing and rehab settings, reconcile medications, and keep your recovery on track until you're safely home.

Recovery doesn't end at the hospital doors. The weeks after discharge are when the gains are won or lost, and they are also when care most often fragments — a new facility, a new therapist, a new set of instructions, and nobody holding the thread.

Post-acute care covers the rehabilitation you may need after a hospital stay, whether that means inpatient care at a facility, outpatient therapy, or medical care in your own home. The goal is straightforward: restore your independence, rebuild your strength, and keep you from going back to the hospital.

Planning starts before you leave

Before you are discharged, we meet with you to evaluate what you will need and agree on the plan for what comes next. If a rehabilitation facility is the right setting, we make the referral and coordinate it. If you are heading home, we make sure the support is in place before you get there.

Hospice and palliative care is one of our own services, led by Dr. Dheeraj Kamra — so if that is the right next step, it stays with the same team rather than becoming a hand-off to strangers.

We follow you the whole way

Your recovery stays under the same set of eyes. We track your progress through in-office visits or telehealth appointments you can join from a computer, smartphone, or tablet, so adjustments happen when they are needed rather than at the next scheduled check-in.
